CAR AUDIO · BLANK RECORD
Noise observation log for a mechanic
Use this page when you already noticed a noise and want a note a mechanic can read. You leave with a blank observation: when it happened, when a similar moment stayed quiet, and the time point in a recording you already have. Download it, copy it, or print it. The page does not find the cause.
Separate what you observed from what you guess. Bring the recording time point you already have.
- 1Write down when it happened.
- 2Keep what you observed separate from what you guess.
- 3Note the conditions when it repeated, and when a similar condition stayed quiet.
- 4Hand this note over with the time point in any recording you already have.
Do not fill this in while driving, and do not make a new recording while driving. A repair outcome belongs on a different page and stays out of this observation. This page does not name a part, decide whether the car is safe, or claim the sound will be easier to reproduce.

Noise observation
From memory, or from a note and recording you already have. Not a diagnosis.
Repair outcome: write it on a separate page. Leave it out of this observation.
When this record is not enough
- You need someone to name a part or decide whether the car is safe to drive. This note does not do that. Take the car to a qualified person.
- You only have a guess and no observation. Write the guess in the guess field, and leave “What was observed” as unknown rather than filling it with the guess.
- The job is a setting you changed, or one chosen piece of audio. Those are the tuning log and the listening comparison checklist.

Questions
Will this page tell me which part is wrong?
No. It is a note for a technician. It does not name a part or a repair.
Should I record a new video while driving?
No. Use a recording you already have. Do not fill this sheet while driving, and do not make a new recording while driving.
Where do I write what the shop did later?
On a separate page. Leave the repair outcome out of this observation so the original note stays what you saw, not what was done afterward.

Editorial responsibility: SALACO. Checked on 15 October 2026. Not reviewed by an acoustic expert or a mechanic.
The steps and blank fields are SALACO’s own record. This page does not report a repair SALACO performed, and it does not use another site’s diagnosis as its own.
